HEALTH OFFICE

Ryan Nena is a free health clinic located in Room 128.  There is a full-time nurse on duty during regular school hours.  It is strongly recommended that you register with Ryan Nena so that your child may receive free health services (physicals, vaccinations, prescriptions). 

If a student becomes sick or injured, he/she will be sent to the health office.  Parents may be requested to come and pick up a student at that time.  P.S. 64 reserves the right to call 911 in the event of a medical emergency. 

If a child has an illness or chronic medical condition it is the parent’s responsibility to notify the school and provide the necessary documentation and medication.  Students who are required to take any form of medication (prescribed by a doctor or over-the-counter) must take the medication to the health office when they arrive at school.  A note from the parent/doctor as to how the medication is to be administered must accompany the medication and a 504 may be required.  Students will report to the health office to take all medication.

Emergency information cards (blue cards) are to be filled out each school year.  During the school year if changes are made at your place of employment, telephone number, or alternate contacts, please call the school office (212) 673-6510, so that the emergency card can be updated.  It is important that all information on the emergency cards is completed accurately.
